CareFlow integrates directly with PointClickCare (PCC) to streamline referral management and patient admission processes. This guide walks you through how CareFlow and PCC work together and the simple steps you’ll follow to ensure a smooth patient sync between systems.
Overview of the Integration
When a referral is marked as Pending Admission in CareFlow, it will allow you to begin the sync process with PCC.
The PCC Sync Card appears in the referral's Overview tab, but users must manually click to push the sync to PCC. (New clarification)
The integration enables automatic creation of patient profiles in PCC and keeps key data in sync between both systems.
Step-by-Step Guide
1. Mark Referral as Pending Admission in CareFlow
Navigate to the referral record in CareFlow.
Change the referral status to Pending Admission.
Once marked, the PCC Sync Card automatically appears within the referral's Overview tab.
Click the Sync to PCC button within the PCC Sync Card in CareFlow.
The sync status will display Pending with this message:
“Please accept patient in PointClickCare so we can sync all patient data.”
2. Accept the Patient in PointClickCare (PCC)
Log into your PointClickCare system.
Go to the External Pending tab
Found under both the Admin Tab and the Clinical Tab.
Choose Create New or Merge Contact depending on whether this is a new or existing admission.
3. Confirm and Save the Patient in PCC
Review the patient’s demographic information.
IMPORTANT: Make sure the SSN is entered in CareFlow before syncing, as this is required to save the patient in PCC.
Do not manually add additional demographic information in PCC—CareFlow will automatically sync the rest once saved.
Simply click Save to accept the patient.
The patient is now created within the New tab within PCC.
4. Sync Status Updates in CareFlow
Once the patient has been accepted and saved in PCC:
CareFlow will automatically update the Sync Status.
All demographic information and files will sync over to the new patient in PCC.
If the sync doesn’t update immediately:
Click the Refresh icon next to Sync Status within the referral.
This forces a manual sync between CareFlow and PCC.
If any new files are added to CareFlow after the initial sync, click the Refresh Sync button to push the new documents into PCC.
5. Patient Profile and Completing the Admission in PCC
The patient will now appear within New under your standard patient listing.
The Edit and ADT (Admit/Discharge/Transfer) buttons will be visible next to the patient’s name.
After the patient is accepted:
Your facility team will need to complete the ADT (Admit/Discharge/Transfer) workflow within PCC.
This step may be performed by the admitting nurse or other designated staff, based on your facility’s process.
Once the ADT is completed, the patient will move from New to Current within PCC.
Quick Tips & Reminders:
Always mark referrals as Pending Admission in CareFlow to initiate sync.
Remember to manually push the sync in CareFlow via the PCC Sync Card.
Accept and save the patient in PCC; ensure SSN is entered in CareFlow first.
Do not manually alter demographics in PCC -- CareFlow will sync them for you.
Use the Refresh icon in CareFlow to sync any new documents added later.
Complete the ADT in PCC to finalize the admission.
